Medical Form Update
Make sure your scout's Medical Form is current.
The Class I Medical Form is required for any scouting activity, and must be updated each year. Simple medical information and a parent's signature is required for this form.
The Class II Medical Form is required for any scouting activity that is longer than 72 hours in duration. Summer Camp would be an example of an activity that requires a Class II medical form. This form requires a doctor's signature, as well as more detailed medical information. The Class II medical form must be renewed every 3 years.
The Class III Medical Form is for high adventure activities, and must be renewed every year. Any scout leader over the age of 40 must submit a Class III medical form to the Troop.
If your scout's medical forms must be renewed, please give the new forms to the Troop Record Keeper, Mr. James May. Without the proper medical information on record with the Troop, your scout will be denied the opportunity to participate in some of the Troop's activities.
